Contract/Policies for Jenny’s Day Care
This contract is made between ,
Parent(s)/Guardian(s), and _________________, Provider, for the care of the
child , date of birth .
1. The fee shall be $120 per week for a child under 18 months old or $110 per week for for a child 18 months old until potty trained or $100 for a child who is potty trained.
2. Care shall normally be provided between the hours of 7:30 am and 5:30 pm. Exceptions to these hours must be discussed and approved in advance, and may be subject to additional charges.
3. The scheduled fee is due in full each week, even if the child does not attend every day, in order to reserve the child’s spot.
4. Fees are due in advance, each Friday before the next week of care. A $10 late fee will be charged if payment is not received by Friday for the next week of care. Additionally, if the late payment is not received by Monday of the week of care, another $10 late fee will be assessed. If payment is a week overdue, the child will be excluded from care until payment in full is received.
5. If the child is picked up after the agreed upon pick-up time without advance approval, there will be a late pick-up fee of $10 for up to 15 minutes late and $5 for each 5 minute increment beyond 15 minutes late.
6. Payment may be made by cash, check, or through one-time or recurring transfers from a bank account through PayPal. Payment may be made by credit cards through PayPal as well, but a 3.15% charge must be added to the weekly fee to absorb the fees PayPal charges for credit card use. There will be a $30 fee for any returned check.
7. Care will not be provided on the following holidays: New Year’s Day, Memorial Day, 4th of July (or the Friday before if on a weekend), Labor Day, Thanksgiving, the Friday after Thanksgiving, Christmas Eve, and Christmas Day. The weekly fee must be paid in full even on weeks including a holiday.
8. If the Provider is unable to provide care because of illness or other emergency, the Provider will be replaced by a substitute qualified to cover a temporary absence. In the rare event that neither the Provider or a qualified substitute is available to provide care, the Parent(s)/Guardian(s) will be notified as quickly as possible and will be required to make other arrangements for child care for that time period at their own expense. Fees will be refunded for any such days when neither Provider nor substitute is available to provide care.
9. Parent(s)/Guardian(s) must submit a complete Medical Record, History of Child Immunizations, Child Health Assessment, and Authorization for Emergency Medical Care for each child before care can begin. The Medical Record, History of Child Immunizations, and Child Health Assessments are transferable and may be available from a prior day care.
10. Children who meet any of the criteria listed on the KDHE handout “Guidelines for Exclusion of Children Who Are Ill” will be excluded from care. The criteria include but are not limited to fever of 101 degrees, vomiting, uncontrolled diarrhea, untreated pink eye, head lice or scabies. The Provider will supply the Parent(s)/Guardian(s) with this handout, as well as with the KDHE recommendations for well-child care, in the Parent Information Packet.

11. Breakfast, lunch and an afternoon snack will be provided during a full day of care. Meals will conform to the USDA’s Child and Adult Care Food Program’s guidelines for healthy meals and snacks. Parent(s)/Guardian(s) should not send special food or snack for the child unless providing enough for everyone to share. Also, Parent(s)/Guardian(s) should inform the Provider if the child has any food allergies so necessary precautions may be taken and appropriate meals and snacks provided.
12. Nap time will be observed each day, for one hour or longer for children who remain asleep. Children who no longer nap will observe a quiet rest period each day. The Provider will provide beds and bedding, but the Parent(s)/Guardian(s) may send a special blanket or stuffed toy for the child if so desired.
13. The Provider has a written discipline policy which is included in the Parent Information Packet.
14. Children over 18 months will be provided with at least one hour of outside play each day that weather permits. Children under 18 months will be taken outside daily when weather permits, though not necessarily for a full hour. Parent(s)/Guardian(s) should dress the child appropriately for outdoor play and send jackets, coats, or gloves as needed.
15. Parent(s)/Guardian(s) should provide diapers or disposable training pants if the child requires them. Wipes are provided by the Provider. Each child should have a complete change of clothes at day care in case of accidents. It is against KDHE guidelines for the Provider to rinse out or wash any clothing that is soiled at daycare, in order to prevent the spread of disease from urine or feces. Soiled clothing will be placed in a plastic bag to be taken home and washed.
16. If the child requires medication (over-the-counter or prescription – even Tylenol, Motrin, Neosporin, or sunblock), the Parent must submit an “Authorization for Dispensing Medications to Children or Youth” form. Medications must be in the original bottle with label and must be administered according to label directions.
17. The child will have access to the Provider’s small dog, a Boston Terrier named Mason, during day care hours. Mason is very gentle and very accustomed to being around children. His vaccinations are up-to-date and a record is on file, available for review.
18. This contract may be terminated by either party at any time and without cause by giving two weeks written notice of termination. Payment is due for the two-week notice period whether or not the child is brought to the Provider for care. Two weeks’ notice is not required when terminating during the first two weeks of care, which are considered a trial period. The Provider may terminate the contract without advance notice if payment is not made as detailed herein.
By signing this contract, the Parent(s)/Guardian(s) agree to abide by the written policies of the Provider. The Provider may amend these policies by giving the Parent(s)/Guardian(s) a copy of the the new or changed policies at least two weeks before they go into affect.
